The MNL48 3rd General Election was held on February 6 – 20, 2021 during ABS-CBN's It's Showtime online and live programs, and MNL48's social media pages. Fans can vote for their favorite member. The voting ballots were included in MNL48's 6th single RIVER.
In contrast with the usual Senbatsu Election, the Top 16 became the 7th Single's Senbatsu (Released a year after the election) while the #17 to #32 will be the 8th Single's Senbatsu (Yet to be released) instead of receiving a coupling song.

- Candidates who withdrew after the voting period began due to graduation or dismissal:
- It was announced on September 30, 2020 that only the Top 36 ranking members will be chosen as the official members of MNL48. On February 13, 2021, it was then clarified that members from Rank 37 to 48 will remain as Kenkyuusei. Official members who belong to this category will be demoted from their respective teams.
- Ella Mae Amat had the biggest ranking increase (No Rank to #5 (REENTER)), while Kyla Angelica Marie De Catalina had the biggest ranking decrease (#14 to #40 (▼26)).
- For members who ranked in the top 48 for both this year and last year's elections, Sandee Garcia had the biggest ranking increase (#48 to #7 (▲41)).
- For newly-ranked members, Francese Therese Pinlac had the biggest ranking increase (No Rank to #23 (NEW)).
